Payments & Insurance

We accept payment by private insurance, HMOs and Medicaid.  A self-pay option is also available.  Upon check-in co-pays and payment for services not covered by insurance or Medicaid is due.  Payment methods accepted are cash, MasterCard, VISA, American Express, Discover, money orders and checks. We have a secure on-line payment page for your convenience.

Prior to the first session with the therapist, every effort is made to contact the insurance company to verify eligibility and benefits.  We encourage you to also contact the insurance company for plan benefits so you will have a clear understanding what your insurance will or will not cover.  Regardless of the information the insurance company shares with us concerning the benefits, it will ultimately be your financial responsibility to pay for any services not covered by the plan.

As a courtesy to our clients, we will file insurance claims for payment.  If there is any remaining balance due to Crossroads, an invoice will be mailed to the appropriate financial responsible party.  If payment isn’t received within thirty days of the invoice date, Crossroads reserves the right to take appropriate advanced measures to collect monies owed.
